Patient Forms

If you are a new patient, please use the link below to complete both the New Patient Form and Medical Update Form prior to your first appointment. This will allow the new patient process to run quickly and more efficiently for both our practice and you.

If you are an existing patient, we also ask that you use this link to complete the Medical Update Form prior to your appointment. This form must be updated every 12 months to ensure we have accurate information.

Your First Visit

Your new patient appointment may include multiple visits to our office. For your initial  consultation, we will schedule time for you to meet one-on-one with Dr. Newman. He will ask you about your dental history and your oral health needs. Dr. Newman will also ask how you feel about your smile, about your goals for your smile, and about your highest priorities for improving your smile. He will listen intently to your input and work with you to create your lifetime dental plan. Your lifetime dental plan is designed to meet your smile goals in a way that respects your priorities and your finances while minimizing the amount of dentistry you will need across your lifetime.

Your first scheduled cleaning appointment will include time for you to meet your hygienist, receive a thorough cleaning and examination, and meet with Dr. Newman about your dental health.

Diagnosis visits may include panoramic x-rays, bitewing x-rays, extraoral photos, periodontal probing, and treatment planning. We will sit down with you to discuss all our findings, recommendations, and prices to accommodate your schedule and financial needs.

Insurance and Billing

We can help you maximize your benefits. We accept most dental insurance plans and are an In-Network provider for Delta Dental.

We recommend that you contact our office prior to your appointment to provide us with insurance information for verification of benefits.

Financing Options

We have many financial options. For your convenience, we currently accept cash, checks, and most major credit cards including Visa, MasterCard, American Express, and Discover. We offer financing through CareCredit, a third party company that offers no-interest payment plans.

We offer a 5% discount for accounts paid in full at the time of treatment.
